I am considering you have already filled up form RFD-01A and want to download a copy for submitting to the GST jurisdictional office. Here is how to download the form:

Login to your username in the gst portal . Use Microsoft Edge for best performance.

Click on the Services Tab and click on Refunds and goto My Saved/Submitted Applications.

You can use the image for reference. Click on Download on the Actions Column of the application and download the form.
