You will need to file the following documents (attachments) along with the application in form FC-3:

Signature of Chief Functionary (max 50kb JPG)
Seal of the association (max 100kb JPG)
Registration certificate of the association (max 1mb PDF)
Memorandum of Association/ Trust Deed (max 5mb PDF)
Activity Report for the past 3 years (Max 3Mb PDF)
Audited statements for the past three years (Max 5mb PDF)
